About Our Client

My Client is a Charity based throughout the North of England. They work with people who have a learning disability, have mental health problems, have issues around substance use - drugs and/or alcohol - or who are or have been homeless.

Services include a variety of housing & accommodation based support and care, day services & community based support, they are currently seeking passionate Support Workers to join their growing team.

Job Description

  • With support from the Manager, to prepare appropriately detailed, outcome focused, person-centred Support Plans with clients
  • To fully review Support Plans on at least an annual basis, or more often as necessary or agreed, taking into account client's changing needs and wishes and amending accordingly.
  • To support clients to enable them to live as independently as possible, in accordance with their particular skills and abilities.
  • To keep appropriate, accurate records, including daily notes, with full involvement of the client.
  • To ensure that all support and care is provided in a way which protects clients from abuse and exploitation, maintains their dignity and supports positive risk taking.
  • Where required to provide personal care to clients in a way which respects their choices and dignity, including assistance with washing, bathing, toileting and continence.
  • To work at all times in accordance with Health and Safety requirements, such as Manual Handling, Food Hygiene, Fire Safety and Risk Assessment procedures; to take personal responsibility for ensuring Health and Safety training is up to date.
  • Awareness and understanding of infection control and prevention procedures and how to implement these as appropriate for the client group.
  • To collect, store, administer and keep records of medication
  • To carry out a range of household duties, in accordance with the needs of the service, including cooking, cleaning, laundry, and other domestic duties as required.
  • To accompany and support clients in attending routine and specialist health appointments, such as doctors, dentist and other visits as required.

The Successful Applicant

  • Understanding of the importance of maintaining accurate written and numerical records

  • Ability to undertake a wide range of personal care and domestic duties

  • Able to work the range of shift patterns required for the service

  • Willingness to undertake relevant training & qualification in care

  • Able to liaise and communicate effectively with carers, families, inspectors and other external stakeholders

Shift: 37.5 hours per week (working between 7.30 a.m - 9.00 p.m including sleepovers and weekends) -
